    I offer premium membership at $5 per 6 months or $8 per year. Here are the features,

    Access to all premium tutorials and resources.
    Removal of all ads and adf.ly links.
    Your own @gfxforums.net email account. (Usable with Windows Live Messenger)
    Can open and close your own threads.
    Ability to edit user title.
    Ability to change user name.
    Can edit and delete your own posts without time limit.
    Ability to give negative reputation.
    Extra user stars.
    4x more attachment space, totalling 40,000kb.
    Can upload avatars from your computers.
    Ability to give more reputations per day, and from posts.
    Can use the PM tracking system.
    Extra inbox space (1000).
    Can view hidden online users.
    Displayed on the forum team page as a supporter
    Purple user name and a premium members user bar
    And few others that aren't listed.

    How do you drive premium memberships on your site? what features do you offer as a reward and have you been successful in making your site self sustaining?

    I had a subscription on my cheating site. It's 5 dollars a month or 20 dollars lifetime.. Almost all of our subs are lifetime and no longer generate money, but the site really is not about the money and it pays for itself. I think that is where a lot of sites fail with this. People try to use it to constantly generate profit for their sites and that only angers members. But, yeah it is definitely a great tool for a site.
