I was wondering if it's possible to ban a specific proxy service. Not the IP addresses, but the service itself. We have a problem member who is trying to return and we've found out from a source. which proxy service she's using.

Any help with this would be great.

That mod will not ban a proxy service. Its designed to extract the real ip of someone using a proxy server, but will only do that if the proxy passes on the details (as http headers). By definition, annonymous proxy servers do not do that.
